* In collaboration with our partners, OCHPP refined the boundaries of the former LHIN-based Sub-Regions – now called Modified Sub-Regions – to remove mismatches and overlaps with smaller geographic units. These adjustments were implemented in July 2025 with the goal to reduce the risk of back-calculation of small cell counts (1-5) and in compliance with privacy guidelines. These areas can be reported in one of two ways: (i) as 76 independent units (n=76);
or (ii) broken out into 78 segments (n=78), where Modified Sub-Region 501, which is split by an Ontario Health Region boundary, is further subdivided so each component is fully contained by one of its two Regional geographies. For more details see our FAQ on Modified Sub-Regions.
